It says "noble" yes, but it never says It uses the Noble stat block. Not all nobles use it either. In fact is give the majority of nobles the commoner stay block, maybe with a bit higher charisma and lower physical stats. (A typical noble lady is hardly going to pose even a CR 1/4 theat.)

Noble (base block) + Expert or Mage (Healer) are generally the most useful builds I’ve seen by popular vote. However, a warrior Ireena could be a very cool vibe as well. Really; it depends on your players party makeup, filling whichever gap is the largest in the party is usually the best way to earn her some big “party mascot” points.

Ireena At Third Level. When the party first meet Ireena in Barovia they should be third level, and Ireena is a scared noble woman, who has been sheltered by her adopted father in his mansion for as long as she can remember. She leaves with the party because her adopted brother wants her to, and because the party seem like they can protect her.
