I really like the metal stand I bought from Caprine Supply. The down side to wood is it absorbs liquids, so wooden stands can eventually take on a pretty nasty smell of old milk. On the plus side, wooden stands are usually cheaper or you can make one yourself. My wooden stand is also very lightweight so it's easy to take to shows and move around.

I put grain in a bucket, usually attached to the same eye bolt as the lead rope, and let the doe eat while I milk her.

They are especially finicky about the rear feet. I hold as close to their body as possible & will even do the other rear hoof from the same side.

Some will. I had a Nigerian doe who would ABSOLUTELY stand and be bred at any time during her pregnancy. At first I was confused, but she had her udder going, I could FEEL kids moving, and she kidded right on time, despite being "bred" again and again in her pregnancy, right up until her forth month.
