Florida certificates are issued through the Bureau of Vital Statistics at the Department of Health. Orders may be placed in person, by mail, or online through VitalChek.
Birth Certificates. The Florida Department of Health in Broward County’s Vital Statistics Offices are committed to providing the highest quality and most expeditious service possible in the provision of birth records. There are three ways to obtain birth certificates.
Death certificates are available from 2009 to the present. Death certificates older than 2009 can be obtained from the State Office of Vital Statistics in Jacksonville at (904)359-6900 Ext. 9000.
INFORMATION AND INSTRUCTIONS FOR DEATH RECORD APPLICATION. AVAILABILITY: Death registration was not required by state law until 1917 however there are some records on file at the State Office of Vital Statistics dating back to 1877.
Any person of legal age may be issued a certified copy of a birth record (except for those birth records under seal) for a birth event that occurred over 100 years ago. BIRTH RECORDS UNDER SEAL

Most birth records between the years 1930 to present can be obtained through this office. Records on birth events that occurred in 1929 or earlier may be obtained from the State Office of Vital Statistics .
