The signs and symptoms of hydronephrosis depend upon whether the obstruction is acute or chronic, partial or complete, unilateral or bilateral.Hydronephrosis that occurs acutely with sudden onset (as caused by a kidney stone) can cause intense pain in the flank area (between the hips and ribs) known as a renal colic.
It is the most common cause of hydronephrosis detected in utero and is the most common anomaly detected on prenatal ultrasounds. [1] [2] It occurs in approximately 1 in every 1500 live births, is most commonly seen in males, involves the left ureter twice as often as the right ureter. [1] [2] UPJ obstruction is transient in most cases. [2]
Pyonephrosis (from Greek pyon 'pus' and nephros 'kidney' [1]) is a dangerous kidney infection that is characterized by pus accumulation in the renal collecting system. [2] It is linked to renal collecting system blockage and suppurative renal parenchymal destruction, which result in complete or nearly complete kidney failure. [3]
Kidney stones are becoming more common with time, and their incidence is believed to increase recently due to unhealthy diet habits. The passage of kidney stones into ureters might lead to their impaction and the development of local inflammatory process around the stone in addition to the obstruction of the ipsilateral kidney and build up of pressure manifested as hydronephrosis.
Patients can also be treated with alpha blockers [7] in cases where the stone is located in the ureter. A 2019 review found three cases of renal colic were hydronephrosis caused by malpositioned menstrual cups pressing on a ureter. When the cups were removed, the symptoms disappeared. [8]
The renal pelvis is the location of several kinds of kidney cancer and is affected by infection in pyelonephritis. [citation needed] A large "staghorn" kidney stone may block all or part of the renal pelvis. The size of the renal pelvis plays a major role in the grading of hydronephrosis.
