Pennsylvania Route 61 Truck (PA 61 Truck) is a truck route of PA 61 in Sunbury that helps larger vehicles avoid a narrow town square area and a tight intersection with PA 405. The northbound truck route splits from PA 61 in the downtown area by heading north on two-lane undivided North 5th Street for two blocks.
The route intersects the beginning of one-way northbound Centre Avenue, which leads to PA 61, and comes to a bridge over Norfolk Southern's Harrisburg Line #2 before it has a junction with the southern terminus of PA 61 at Greenwich Street. The road narrows to two lanes and continues north, lined with homes.

The section currently under construction continues south from the intersection with U.S. 15. This section will carry U.S. 15 and PA 147 concurrently south to a new trumpet interchange, from which a new spur of the highway will extend PA 61 from the Veterans Memorial Bridge to the CSVT. This spur will also carry a concurrency with PA 147.
