The Human Centipede 2 and it’s not even close. I’ve seen the two you mentioned, as well. I chase the worst of the worst. As far as awful, depraved and absolutely disgusting, it doesn’t get worse. That movie doesn’t stop hitting you until it’s over. When you think it can’t get worse, it does.
Some middle ground movies to check out, these are a bit more gory but all very disturbing. “The Woman” (2011), its sequel. “Darlin” (2019) “Raw” (2017) If you’re in the mood for sheer depravity aren’t overly concerned about the quality of the movie both “Trauma” (2017) and “Grotesque” (2011) are horrific watches.
Tier 1 is mainstream / classic horror films. Tier 2 is more graphic mainstream horror. Tier 3 is notorious “disturbing” films. Tier 4 is pseudo-snuff and exploitation films. Tier 5 is very extreme art films and more pseudo-snuff. Tier 6 is mondo films. Tier 7 is fetish films. Tier 8 is shock compilations and generally morbid shit.

It's an exploitation film despite pretending not to be (I hate it), but it's also uber disturbing not just with the events depicted but with the power of extreme immersion. Absolutely one of cinema's biggest endurance tests.

John Waters, Matthew Gray Gubler Ariel Winter and Roger Bart were good but this movie stuck with me and not in a good way. There was A LOT of blood scenes, but not in the context you would be used to in a horror movie. Squelching is a common thing. This has to be one of my most disturbing movies up there with Human Centipede II!
