Hierarchical structure can be tall or flat (height of the pyramid for example), it still stays a hierarchical structure. So being flat is an optional property of a hierarchical structure (althrough hierarchical structures tend to be tall rather than flat). Also organisation can not be "flat". It can only have flat organisational structure.
