It is counterintuitive because you are not actually setting up a new account. But once you click to add a new account you can choose to sign in with your Microsoft account in the Launcher. Once you do that, it will automatically use the Microsoft account to log you in from the Launcher. And my worlds were there without change.

I had my computer broken for some time and just got it fixed, tried logging to minecraft java launcher with old mojang account and I can't, like it doesn't exist.
