The highway begins at an intersection with US 64 and Highway 1 in northern Wynne in the Arkansas Delta. It runs due south as a section line road through an industrial and agricultural area to John Brown Road, where state maintenance ends. The route shares the bridge with Interstate 55 (I-55) and US 61, US 70, US 78, and US 79. The route traverses several streets in Memphis before becoming a rural divided highway in eastern Shelby County.
US 65 passes through Greenbrier, Clinton, Marshall, and Valley Springs. US 65 will join US 412 and 62 before passing through Harrison. After leaving Harrison, US 412 and 62 separates from US 65 at a freeway-to-freeway interchange. US 65 becomes a divided highway. US 65 continues north to the Missouri state line and enters Missouri.
