The northern segment of SR 37 begins at a junction with I-469 on the northeast side of Fort Wayne. From there it runs northeast approximately 20 miles (32 km), passing through Harlan, to terminate at the Ohio state line near the Allen–DeKalb county line. The road continues northeast in Ohio as State Route 2, to Hicksville, Ohio and beyond.

It originally comprised two communities, the other of which is now Fort Wayne's Maysville neighborhood. The border between the two towns was Georgetown Road (Indiana State Road 37), with Harlan being on the north side of the road. [6] The post office at Harlan has been in operation since 1851. [7] Maysville faded from existence soon afterwards.
