Pulp necrosis is a clinical diagnostic category indicating the death of cells and tissues in the pulp chamber of a tooth with or without bacterial invasion. [1] It is often the result of many cases of dental trauma , caries and irreversible pulpitis .
The overlying skin may appear shiny and tense as well. [8] Later signs more suggestive of necrotizing changes (but only present in less than half of cases) are: Bullae (blisters) Crepitus (palpable gas in tissues) Reduced or absent sensation over the skin of the affected area [2] Ecchymosis (bruising) that progresses to skin necrosis. [2]
In a periapical abscess, usually the origin is a bacterial infection that has accumulated in the soft, often dead, pulp of the tooth. This can be caused by tooth decay, broken teeth or extensive periodontal disease (or combinations of these factors). A failed root canal treatment may also create a similar abscess.
The pulp cannot recover from the insult and damage. For example, decay that has reached the pulp of the tooth introduces bacteria into the pulp. The pulp is still alive, but the introduction of bacteria into the pulp will not allow the pulp to heal and it will ultimately result in necrosis, or death, of the pulp tissue. [11]

The signs and symptoms of pericoronitis depend upon the severity, and are variable: Pain, which gets worse as the condition develops and becomes severe. [2] [9] The pain may be throbbing and radiate to the ear, throat, temporomandibular joint, posterior submandibular region and floor of the mouth. [2] [4] There may also be pain when biting. [9]
