1. Determine the rhythm. The time from one P wave to the next P wave is the P-P interval, while the R-R interval is the time between one R wave and the subsequent R wave. A regular rhythm is determined when the P-P interval and R-R interval are consistent across the EKG tracing.

The goal of reading an EKG rhythm strip is to determine the rate and rhythm of the patient. This is great for identifying baseline cardiac rhythm as well as any arrhythmias or ectopy that may occur (like a premature beat).
The rhythm is best analyzed by looking at a rhythm strip. On a 12 lead ECG this is usually a 10 second recording from Lead II. Confirm or corroborate any findings in this lead by checking the other leads. A longer rhythm strip, recorded perhaps recorded at a slower speed, may be helpful.
