The first Mass was celebrated in the completed building on 12 May 1850. In June 1859 it became the cathedral for the first Bishop of Brisbane. It is the oldest Catholic church in Queensland. Old St Stephen's School is a three level building facing Charlotte Street. This building was the location of St Stephen's School until the 1960s.

The church was named Mary Immaculate and situated where the Marymac Community Centre now stands. The first parish priest was Fr James Gallagher who transferred from St Joseph's Cathedral in Rockhampton. The church was a timber structure surrounded by a verandah and was able to hold 300 people. With the growth of Brisbane's Catholic community and with Separation approaching, the Roman Catholic Archdiocese of Brisbane was created in 1859. Old St Stephen's became a cathedral but its capacity became increasingly inadequate and on 26 December 1863, Bishop James Quinn laid the foundation stone for a new cathedral.
