The following is a list of characters that first appeared in Doctors in 2021, by order of first appearance. All characters are introduced by the programme's executive producer, Mike Hobson. Luca McIntyre (Ross McLaren) made his debut in February as a nurse at the Mill. In March, PPG chairwoman Miranda Evans (Ruthie Henshall) began appearing.
