Since 2002, the USAMO has been a six-question, nine-hour mathematical proof competition spread out over two days. (The IMO uses the same format.) The test for Grades 9-12 covers algebra I and II, geometry, trigonometry, math analysis, analytic geometry, pre-calculus, and elementary calculus.
