Mar 3, 2010. #8. 2 er visits same day. We also combine the 2 visits. The CMS rule regarding paying 2 visits/ same specialty/same day does usually hold true unless the visits are totally unrelated. We look at both visit notes and combine the time and documentation and choose our level of E/M by that.

Best answers. 2. Jan 26, 2011. #2. yes you will need a 25 modifier on the E&M any time there is a status S or T procedure performed in any session of the day. So if you have a 510 99213 and a 450 99284 with the 450 99372 on the same date of service then both E&Ms will need a 25 modifier. H.
Mar 13, 2009. #1. pt. presents with ear pain and the final diagnosis is Otitis Media. MD does a comprehensive HPI and Exam and then only thing done was a prescription was written so now MDM is moderate. According to my coders the level is now 99284 due to the documentation however, I disagree from prior coding experience I would code this a 99283.
