Atrophic glossitis is a non-specific finding, [16] and has a great many causes, usually related to iron-deficiency anemia, pernicious anemia, B vitamin complex deficiencies, [16] unrecognized and untreated celiac disease (which often presents without gastrointestinal symptoms), [17] [18] [19] or other factors such as xerostomia (dry mouth).
In most of those cases, people had long been told they were allergic to penicillin — usually after developing a rash within several days of taking the antibiotic as a baby or toddler.
Between 3 and 10% of children taking amoxicillin (or ampicillin) show a late-developing (>72 hours after beginning medication and having never taken penicillin-like medication previously) rash, which is sometimes referred to as the "amoxicillin rash". Bacterial rashes can be diagnosed by testing pus from a lesion along with a blood sample. Bacteria can be seen under a microscope with a special stain or may be found on a culture . Fungal infection with Candida may also cause a similar rash in newborns, but it usually causes additional symptoms like thrush.
Of course this supposes that someone reporting an allergy really has had an allergic reaction to the drug - the rash could have been from the disease itself (i.e. is a rash that develops with a sore throat promptly treated with a penicillin from the drug, or was the rash about to appear anyway as part of the disease i.e. scarlet fever).
