While the exact cause or causes are unknown, Crohn's disease seems to be due to a combination of environmental factors and genetic predisposition. [96] Crohn's is the first genetically complex disease in which the relationship between genetic risk factors and the immune system is understood in considerable detail. [ 97 ]
In developed countries, 10–15% of adults experience gallstones. [4] Gallbladder and biliary-related diseases occurred in about 104 million people (1.6% of people) in 2013 and resulted in 106,000 deaths. [8] [9] Gallstones are more common among women than men and occur more commonly after the age of 40. [2]
When other causes were recognized, and an idiopathic, primary form described, [23] a classification into three types was proposed: [24] Type 1: Bile acid malabsorption, secondary to ileal resection, or ileal inflammation (e.g. in Crohn's disease) Type 2: Idiopathic bile acid malabsorption, Primary bile acid diarrhea
Obstruction can also be due to duodenal inflammation in Crohn's disease. [7] A gallstone may get lodged in the constricted distal end of the ampulla of Vater, where it blocks the flow of both bile and pancreatic juice into the duodenum. Bile backing up into the pancreatic duct may initiate pancreatitis. [8]
Synthesis of bile acids is a major route of cholesterol metabolism in most species other than humans. The body produces about 800 mg of cholesterol per day and about half of that is used for bile acid synthesis producing 400–600 mg daily. Human adults secrete between 12 and 18 g of bile acids into the intestine each day, mostly after meals.
