    A toilet flange is supposed to set on top of the finished floor (no gap or space). Then the flange must be screwed through the finished flooring and into the sub floor. You can get away with a flange being slightly recessed below the floor by using a thicker wax ring or the wax free seal you mention.

Yep: you attach the bolt to the flange with a washer and nut, and then after installing the toilet, secure it with the second washer and nut, then put the cap on top after sawing off the excess bolt length (or dremeling it, or using a bolt that snaps off).

    Once you glue the flange on the flange hub will also be angled thus throwing the flange itself out of level. The flange and pipe in essence become one when solvent welded therefore if the pipe is out of plumb then the flange will be out of level. Your first instinct to bust up the concrete around the toilet drain is the correct one. Good luck
