On radiological studies, a pleural pseudotumor is visualized as a biconcave or lenticular lesion using conventional chest x-rays and CT scans. The lesion is most commonly located in the minor (horizontal) fissure of the lung. A pleural pseudotumor is also associated with the presence of dependent pleural effusions. [9]
CT scans can more readily differentiate whether pleural thickening is due to extra fat deposition or true pleural thickening than X-rays. [3] If a fibrothorax is severe, the thickening may restrict the lung on the affected side causing a loss of lung volume. [7] Additionally, the mediastinum may be physically shifted toward the affected side. [3]
Intrapleural fibrinolysis with urokinase decreased the need for surgery but there is a trend to increased serious side effects. Although most cases of pulmonary toxicity in medicine are due to side effects of medicinal drugs, many cases can be due to side effects of radiation (radiotherapy). Other (non-medical) causes of pulmonary toxicity can be chemical compounds and airborne particulate matter.
The results from VATS-based pleural abrasion are slightly worse than those achieved using thoracotomy in the short term, but produce smaller scars in the skin. [ 12 ] [ 15 ] Compared to open thoracotomy, VATS offers a shorter in-hospital stays, less need for postoperative pain control, and a reduced risk of lung problems after surgery. [ 15 ]
